Once the patient sign-in form has been completely filled a copy of the completed sheet is available behind special backing paper that gives the office a daily patient log.
All information requested to be completed on the sign-in sheet conforms to HIPAA regulations of not displaying medical information in a public facing manner. Information that can be collected is the date, patient’s name, arrival time, appointment time and who the appointment is with. As a convenience to the office staff we’ve included a box to indicate changes to address and insurance.
