These 8-1/2" x 11" white confidential patient sign in logs include 20 numbered removable sign in shingles to collect patient sign in information while protecting patient confidentiality. Our BF-PSL-20 confidential patient sign in log have a 3 hole punch on the left to allow the forms to be secured inside a 3 ring notebook, blue ink, 100 per Box (50 in each inner box).
