Description:
Unit sets are multiple part, non-continuous forms such as physician's order forms and patient valuables envelope used in medical facilities such as hospitals, clinics, urgent care, and rehabilitation centers. These 8-1/2" x 13-5/8" Patient Valuables Envelopes (white) come pre-printed, include 15 lines to list patient valuables and have a tear off numbered receipt to provide the patient with a record of the valuables received. Our BF-HFS 8 envelopes printed on .002 mil Polyolefin plastic are tear resistant, waterproof and have a peel & seal closure, no special gum/glue/remoistening required. Patient Valuables Envelopes are printed with blue ink and come packaged 500 per box (100 per polywrap).
