Description:
Unit sets are multiple part, non-continuous forms such as physician's order forms and patient valuables envelope used in medical facilities such as hospitals, clinics, urgent care, and rehabilitation centers. These 8-1/2" x 11-3/8" Patient Valuables Envelopes (white) are numbered and come pre-printed to identify and list patient's valuables during a hospital stay. Our BF-HFS 7 envelopes have 4 parts, part 1 & 2 become copies and part 3 & 4 are pocket glued to form envelope with peel & seal closure, no special gum/glue/remoistening required. Patient Valuables Envelopes are printed with blue ink and come packaged 500 per box (100 per Polywrap).
